CHW Booth – Chelsea Hospital Community Health Workers (CHWs) will be offering information and flyers to spread awareness of the CHW Program. A CHW will be there to discuss the CHW Program, meet with community members, and connect community members with resources they may need. CHWs work with patients and community members to overcome barriers to achieving health and wellness by ensuring individuals have basic needs for health and safety – such as housing, food, transportation, education, employment, and other social needs.
